Interactive Media Design Student Forms & Resources

IMPORTANT NOTICE

The Interactive Media Design program has been permanently suspended and no longer accepts applications for admission. Students currently in the major are not affected. Contact the IMD academic advisor if you have questions.

Forms

  • BIMD 498/499 Registration Request. Once you have discussed your research or independent study plans with your instructor, fill out this form to request registration for the credit. Please allow up to two weeks for processing.
  • IMD Major Petition
    • Use this form to request a substitution for a major requirement. You must submit a syllabus, a detailed letter from your instructor, or a similar document that thoroughly explains the content of the course. For General Education requirements (Writing, Diversity, Arts & Humanities, Social Sciences, Natural Sciences), contact your advisor instead of submitting a petition.
    • Please allow up to two weeks for processing. Forms are not processed during inter-quarter breaks. Petitions made near the end of a quarter may take longer to receive a response.
