Facilities Use Request Form
The facilities use approval committee (FUAC) form is to be submitted when facilities at the UW Bothell are requested for non-curricular use. The purpose of the activity(s), seminar, conference and/or event must conform to the UW Bothell mission, to general policy outlined in WAC 478-136, and to UW Bothell policy on use of UW Bothell facilities. If you are unfamiliar with the content of the WAC or UW Bothell campus facilities usage policy, please read them prior to completing this form.
Requests for facilities use associated with curricular events should be submitted directly to the UW Bothell Registrar's Office (Room Request form).
Complete all fields with the requested information. Incomplete forms will not be processed and will be returned for correction.
In order for us to meet your needs, this form must be submitted three (3) calendar weeks prior to the requested use date. Requests made with less lead time will be accommodated, as is possible, based on time, staff and equipment availability.
Campus Security may require you to retain their services for your event, based on the total number of participants, the serving of alcohol at the event, etc. This additional expense will be the responsibility of the Event Patron.
By submitting this form, you agree that you have read the policy on Use of University Facilities and that you and/or your organization meets all of the requirements stated therein. This is a REQUEST for space; please do NOT advertise this event until you have received a confirmation email from email@example.com .
I have read and understood that I am submitting a request for space and that this is NOT a reservation confirmation of said space. If you do not receive an official confirmation email within five (5) business days of submitting this request, please contact the Events office at firstname.lastname@example.org .
